weeks and a total of 5 after the baby is born.
Milk and vitamins are available free of charge to:
an expectant mother and all children (including foster children) under school age in families who are getting supplementary benefits, family income supplement or whose incomes are low. See "Low income entitlement" on page 37.
an expectant mother who already has two children (including foster children) under school age, regardless of family income.
all but the first two children under school age in families with three or more children (including foster children) under school age, regardless of family income.
Free milk is also available for:
handicapped children aged 5 to 16 who are not registered pupils at a school or special school (7 pints per week).
children attending an approved day nursery or play-group or with an approved child-minder (pint each day they attend).
How to claim
A woman who has two children under school age can claim free milk and vitamins when she becomes pregnant. Claim form FW8 from her doctor, health visitor or midwife.
A family with three or more children under school age can claim free milk and vitamins but not for the first two children. Claim form FW9 from local social security offices.
A family receiving supplementary benefit and which includes an expectant mother or children under school age can claim if they are not already getting free milk and vitamins. Claim form A9 issued with their supplementary benefit order book.
A family receiving family income supplement and which includes an expectant mother or children under school age can claim if they are not already getting free milk and vitamins. Claim by letter to Department of Health and Social Security, Family Income Supplements, Poulton-le- Fylde, Blackpool FY6 8NW.
Families not getting supplementary benefit or family income sup- plement can claim on the grounds of low income. See "Low income entitlement” on page 37. Claim form M11 from post offices and local social security offices, maternity and child health clinics.
Claims can be made for a handicapped child who is between the ages of 5 and 16 if not a registered pupil at a school or special school. Claim form FW20 from local social security offices.
Approved child minders and people who organise approved day nurseries or playgroups can apply for reimbursement of the cost of milk supplied to children in their daily care as follows:
in England-to the Director of Social Services of the local authority with which they are registered under the Nurseries and Child Minders Regulation Act 1948 or the Department of Health and Social Security, Government Buildings, Honeypot Lane, Stanmore, Middlesex HA7 1AY. in Wales--to the Director of Social Services as above or the Welsh Office, Pearl Assurance House, Greyfriars Road, Cardiff CF1 9LL.
