The preliminary import notice shall show:

(a) the name and address of the importer;

(b) description of the product it is proposed to import, with

(c)

the following details:

trade name

CCT Number

Reference number in the nomenclature of goods of the

national statistics for foreign trade,

country of origin

country of export;

whit

shown

the cif price it and the total free-attortier of the

Community-price as well as the quantity of the product,ven

in the trade;

in terms of units usually use

(a) the expected date of import.

3. A pro forma invoice signed by the exporter shall be attached

to the preliminary import notice.

4. The importer shall sign the preliminary import notice certifying

that the information given and the attached invoice are correct.

5. Member States may ask for additional information.

The Commission

after thing duly heard the opinions expressed within the Committee provided for

Xxxuxdowa in Article 9, may act similarly, or waive the need to

supply certain information or to produce a pro forma invoice when

the nature of the products or other particular aspects of the

proposed transactions require it.
