This form is for use if you are 16 years of age or over and wish to
apply for a five-year United Kingdom passport (fee 30/-). If you are
under the age of 16 you will need Form "B". If you require a British
Visitor's Passport, do not use this form but obtain a Form "VP " from

PLEASE GIVE THREE WEEKS' NOTICE WHENEVER POSSIBLE
Having completed the form, first check that you have inserted your name
at the top of this page and that you have enclosed two passport
photographs (one countersigned on the back), a completed and signed
personal description slip, the fee of 30- and such documents (e.g.,
birth certificate, marriage certificate, previous passport, etc.), as
are specified in the section headed DOCUMENTS TO BE PRODUCED on page 2
of the NOTES.
Then, if you live in England, Scotland or Wales, take it or post it to
one of the Passport Offices at the addresses given below, preferably to
the one for your area. Alternatively, provided you do not require your
passport urgently (ie., within 10 days), you may sakšit to any Local
Office of the Ministry of Labour.

Form "A" Application for a United Kingdom Passport for
Persons over sixteen years of age
United Kingdom passports are issued primarily to British Subjects who
are citizens of the United Kingdom and Colonies, but may in certain
circumstances be issued to British Subjects without citizenship and to
British Protected Persons. Citizens of other Commonwealth countries
should apply for passport facilities to the appropriate High
Commissioner in London.
NOTES
The foe for a passport is 30/-, payable at the time of application.
Cheques (not normally acceptable for passports required urgently) should
be made payable to the Accountant, Passport Office. Notes should not be
sent unless registered. A new passport is required by a woman on
marriage and the fee of 30/- must be paid. Where the husband and wife
are included on a joint passport the fee covers both.
HOW TO COMPLETE FORM
1. Sections 1, 4, 5 and 8 of this form and the personal description slip
must be completed by all applicants for new passports.
Sections 2 and 3 must also be completed by thom to whom these sections
apply.
Sections 6 and 7 must be completed if the applicant's wife and children
under 16 are to be included in the passport, A joint passport for
husband and wife cannot be issued if the wife is not British. See Note 4
(0)
IN B-A joint passport cannot be need by the wife when travelling without
her husband.)
The form and personal description slip should be clearly written in the
applicant's own handwrung using block capitali for all names,
British women who are getting married and wish to travel abroad
immediately after the wedding should apply loa Passport Office to obule
information and the rolevint forms for completion.
Children over 16 years of age may not be included in an adult's
passport.
Application for a seperate pemport for a child waler 16 should be made
on Form "B".
2. Authentication of Application. The application must be countersigned
in Section 9 by a Member of Parliament, Justice of the Pisce, Minister
of Religion, Medical or Legal Practi- tioner, established Civil Servant,
Bark Olsscer, Public Official, Police Officer or any person of similar
standing who has been personally acqumard with the applicant for at
least two years. "A member of the applicant's immediate family is not
acceptable (See also Note 3,3
3. Photographs. Two copies of a recent photograph of the applicant find
also two of his wife where a joint passport is required) must be
included with the application. These muK1 be taken full face without
bat, and must not be mounted. The size must not be more than 2) laches
by 2 laches or less than 2 lacker by 11 inchon. They must be printed on
normal thin photographic paper and not glazed on the reverse side. The
person who countersigns the application is also required to midorse the
reverse side of one copy with the words: "1 certify that this as a true

Where joint passport is required, one of the wife's photographs should
be similarly certified.
In cases where children to be included in a passport are between 14 and
16 years of age and proceeding to the United States of America, two
photographs of each, certified as above, must be furnished.
All photographs, included with an application become the property of Her
Majesty's Government when it is lodged.
4. Documents to be produced.

relationship to the child should be stated in either caso (e.g., father,
mother (widow, since remarried) ).

NE-The mother, or any other person, claiming legal emstody during the
lifetime of the father, must produce the court order committing the
child to bar or that person's custody.
(A change of name (see footnote (i) other than by marnage or adoption
must be substantiated by the production of evidence showi that a bone
file change has been made for all purposes. The original name is not
required in cases of adoption.

documenti personally for examination as any of the Passport or Ministry

with whom arrangements for payment for the passport would be made.
Applicants resident in Northern Ireland may submit Applications
personally, except on Saturdays, at the Foreign O Pasport Agency, 1 May
