4. Address at which notices may be served. 四、可將通知書送達之地址

5. Whether the applicant intends to prospect on his own account or as an employee of any other person. 五、申請人是否以自行經營性質而探錶,抑或受僱於人而爲之?

6. (1) If he is in the employ of any other person, state the name, nationality and address of such person. 六、(一)倘係受僱於人者,請述該人之姓名、國籍及地址

(2) If the application is made by an employee evidence of the employer's consent to the application. (二)倘係受僱於人者,並提出證據以証明僱主同意作是項申請

7. Whether the applicant has previously made an application for a prospecting licence, and, if so, whether any such application was refused. 七、申請人以前會否申請探鑛牌照,若會申請者,該次是否不獲批准?

8. A plan of the area he intends to prospect. 八、申請人擬探鑛之地區圖則
