You are warned that any material falsification or omission of information may result in the Commissioner's refusal to grant a licence.

If the Commissioner decides to grant a licence, a fee of $3,000 will be payable to the Commissioner if the number of tables does not exceed 15, together with a further fee of $2,000 for every additional 5 tables or part thereof, exceeding 15 in number.
