If the application is on behalf of a company, complete Form 7. If the application is on behalf of a club or society registered or exempted from registration under the Societies Ordinance, complete Form 8. If the application is not on behalf of a company or such a society or club, complete Form 6.

You are warned that any material falsification or omission of information may result in the Commissioner's refusal to grant a licence.
