Notice to dissenting shareholders

Pursuant to paragraph 3 of Part 1 of the Ninth Schedule

Please complete legibly, preferably in black type, or bold block lettering.

Now therefore the transferee company in pursuance of the provisions of section 168 of, and Part 1 of the Ninth Schedule to, the Companies Ordinance hereby gives you notice as follows:
