Such application shall be in Form 9 in the Second Schedule. If the Registrar so requires the assignee shall furnish a declaration in Form 10 in the said Schedule.

Where no such joint request is made any person who has become entitled to a registered trade mark by assignment, transmission or other operation of law may request the Registrar to enter his name in the register as proprietor of such trade mark.
