That I did on the day of 19 send to each contributory mentioned in the list of contributories made out by the [Official Receiver and] liquidator on the day of 19 and now on the file of proceedings of the above-named company, at the address appearing in such list, a notice of the time and place of the appointment to settle the list of contributories in the form hereunto annexed, marked "A", except that in the tabular form at the foot of such copies respectively I inserted the number, name, address, description, in what character included and (b) the amount called up and the amount paid up in respect of the shares (or interest) of the person on whom such copy of the said notice was served.
2. That I sent the said notices by putting the same prepaid into the post office at before the hour of o'clock noon on the said day in the
