Note. This form must be submitted in duplicate, with any invoices, bills of
lading, shipping orders, certificates of age or origin and other documents relating to the goods.
One copy only will be issued, and after use must be deposited in the general bonded or licensed warehouse to which the goods are taken. or if it cannot be used, must be returned at one to the office of the 'Superintendent.
