THE TRADE MARKS ORDINANCE, 1906. Application for Registration of Trade Mark under Paru- graph 5 of Section 8.

Representation to be fixed within this square. Three facsimiles corresponding exactly in all respects with the representation affixed to this form to be sent in on separate sheets. Representations of a larger size may be folded, but must then be mounted upon linen and affixed hereto.

NOTE. When the declaration is made by a person who does not understand the English language, the require- ments of The Statutory Declaration Ordinauce 1893 must be complied with, where the declaration is made in Hong- kong, and if made elsewhere a proper clause must be added.
