A request for confiscation or forfeiture shall also include:

(a) the original or a certified copy of any order or judgment

of a court in the Requesting Party in relation to the

proceeds of drug trafficking and a summary of the grounds

upon which the order was mada;

where the person against whom the order is made did not

appear in the proceedings which resulted in the

aforementioned order, a statement that the person had

been given notice of the proceedings in sufficient time

to enable him or her to defend them/

a statement that neither the order nor any conviction to

- which it relates is subject to appeal;

a description of the property to which measures of

confiscation or forfeiture are to be applied including,

as far as possible, the location of the property;

(e) such further evidence as may be required by the Requested

Party.
