(a) In accordance with CSB Circular No. 12/87, I wish to join the New Pension Scheme governed by the

Pension Benefits Ordinance 1987 and the Pension Benefits Regulations 1987. I fully understand that the Pensions Ordinance and Regulations (Cap. 89) will cease to be applicable to me upon my transfer to the New Pension Scheme.
