The Authority has imposed by notice in writing to the
exemption holder on the
day

in relation to this exemption terms and conditions in accordance with
section 22 of the Ordinance. This exemption shall continue in force
subject to the observance of the terms and conditions which are
contained in the said notice and reproduced below:
