It would entail much loss of time and some expense to move the central office outfit. and force from village to village as the work progressed.
The area selected should, wherever possible, include a population something like that of Peter's Hall District (14,000), and this is probably the limit which can be cared for by one Supervising Medical Officer and clerical force.
You have likely noticed that at Meadow Bank no negative results, in the re- examinations after intensive treatment, have been accepted unless the specimen of fæces was secured at least six days after the last thymol was administered.
We recognize the possibility that there might remain ankylostomes in the bowel which, because of the toxic of the thymol, would not be producing ova, and, unless a sufficient interval was allowed after the last treatment, there would be a possibility of error.
For the same reason I would suggest, in the re-examinations of patients in area B who have been on daily treatment, that two re-examinations be required before the patient is pronounced cured.
The first re-examination, which will come at the end of three months' treat- ment, would show some positives and some negatives, and would be made without Those found positive by the first examination would interruption in treatment.
be kept on thymol; those found negative by the first re-examination should have their thymol discontinued at once, and after at least one week's interval without thymol they should be re-examined to find if they femain negative.
This may seem a rather complicated procedure, but only by such an arrange- ment can the element of doubt as to negative findings be removed, and also it pro- vides that there should be no interruption in the treatment of any except those who are likely cured.
Hoping that the above rather lengthy communication may have touched upon some points of interest to you,
The Honourable
The Surgeon-General,
I have, &c.,
