in any one area under & Health Officer is very small and in a large proportion of these places there is already a dispensary which is the real nucleus of the proposed system.
On the other hand, although we propose that the Health Officer should be responsible personally for discovering defects or abuses, he must have an agent, and also, in view of the terms of the law, a witness, whom he can appoint to see that his orders are carried out. We accordingly recommend that there should be a Sanitary Inspector at the headquarters of each Health Officer's circle available for such duty as he may be given by the Health Officer. Two Inspectors are required in the straggling suburban area of Plaines Wilhems. A Sanitary Inspector should be attached to each of the Hookworm Officers. None of the Inspectors should have a local range other than the whole circle. A special arrange- ment is necessary in Port Louis, where the Medical Officer of Health will in future have charge also as Port Health Officer, and while the Government is responsible for scavenging and con- servancy. We propose ordinarily to attach to this circle as assistant to the Medical Officer of Health the additional Medical Officer whose appointment we recommend as a leave reserve. Each of these officers should have a Sanitary Inspector, and in addition there should be a Senior Sanitary Inspector, who will be available for any duty to which the Medical Officer may depute him, but who will ordinarily be made responsible for sanitary arrangements in the Port area. The Director accepts our scheme and agrees that the posts of the four junior Sanitary Inspectors and all the Sanitary Guards may be abolished.
17. Our recommendation is therefore that the superior medical staff should consist of 22 officers, besides the new Superintendent of Prisons, namely, the Bacteriologist, Pathologist and Analyst, three Superintendents of Hospitals and five Resident Medical Officers, the Medical Officers of Health of Port Louis and Plaines Wilhems, three Hookworm Officers and six Government Medical Officers. To these 22 officers we would add an extra post as leave reserve, and the incumbent should be at the disposal of the Director ordinarily as Assistant Medical Officer of Health at Port Louis.
The composition of the directing staff has been the subject of some recent controversy without any very real difference of opinion. The question has practically been decided, and we agree that the Director should be helped by a Deputy Director, on a rate of pay above that of the other officers, who should be specially responsible for the economical management of the hospitals. He would in fact be a Deputy Director (Medical). A Deputy Director (Health) is not required. The total number of superior Medical Officers will thus be 25, apart from the Superintendent of Prisons. The Sanitary Engineer will continue to be attached specially to the anti-malaria works. We have placed our complete scheme before the Director, who concurs in our recommendations.

18. It remains for us to summarize the financial aspect of our recommendations.

Four of these posts are vacant, while the post of Chief Sanitary Inspector is occupied on a temporary basis by an officer who has already reached pensionable age and should therefore be re- tired in the normal course. The Sanitary Warden has also reached pensionable age and should be retired if he is not quali fied to occupy any of the new posts which we propose to create. The present Port Health Officer and the Government Medical Officer, Plaines Wilhems, can be employed in vacancies which will shortly occur. We have already stated our view as to the future employment of the present Radiologist. No special expenditure on pensions is involved by this part of the scheme.

The saving as against the nine posts to be abolished would be Rs.22,200, on the basis of maximum pay. The rates of pay which we assume for the new posts and for the existing posts which would be maintained under our scheme rest primarily on the rates of pay recommended by the Director, namely, Rs.10,500-500-13,500 for the higher specialist posts and Rs.7,000-10,000 for the lower posts; but these rates are propor- tioned to the existing rates of pay of the services and must be
