(a) his name;

(b) his identity card number, if any: (c) bis date of birth;

(d) the date which, in the latest medical examination report received by the senior in- dustrial health officer under paragraph (4) of regulation 16C in respect of the person, is specified as the date on which he first com- menced to work underground in the indus- trial undertaking or us the proposed date on which he was to first commence so to work; and

(e) the nature of his occupation,

and shall produce for inspection to such member the latest certificate issued under paragraph (4) of regula- tion 16C in respect of the person so employed.

(2) No-

(a) proprietor of an industrial undertaking to

which this Part applies; or

(8) public officer,

shall disclose to any person any medical information which is obtained on the examination of a person in accordance with paragraph (3) of regulation 16C, unless-

(i) the person to whom the information relates

consents to the disclosure:

(ii) the information is disclosed to a public

officer acting in the course of his duty; or (i) the disclosure of the information is permitted

or required under any enactment.".

Regulation 44 of the principal regulations is amended. in paragraph (a), by inserting after "regulation” the following-

**16F(2),”.

10. Regulation 45 of the principal regulations is amended, in paragraph (5), by insecting after "regulation" the following- “160(1), 160(2), or".

11. Regulation 46 of the principal regulations is amended,

in paragraph (2), by deleting "or 15" and substituting the follow- ing-

", 15, 15A or 16B".

12. The Schedule to the principal regulations is amended by deleting "SCHEDULE." and substituting the following-

"FIRST SCHEDULE”.

13. The principal regulations are amended by adding, after the First Schedule, the following new Schedule---
