34. The registration officer shall, in the forms to be used in respect of or in connexion with the compilation of the provisional register to be compiled on or before the 16th day of November 1966 and of the final register to be compiled and published on or befor the 1st day of April 1967, make all such alterations as are necessary to adapt them to suit the circumstances in which they may be used under these regulations.

Please read the following notes carefully before completing this Form. Note J. Name and address In English are to be completed in BLOCK LETTERS. Note 2. You may be eligible to claim registration under more than one category.
If so, choose the one under which you prefer to be registered, Note 3. Insert in the appropriate place in this Form, the information required
against the chosen category.
Note 4. It is your responsibility to ensure that the particulars you enter on this Form are correct. Your attention is drawn to the DECLARATION ∞ paragraph 3 of this Form, which you are required to sign. If you knowingly and wilfully make or sign that Declaration which is under in any material particular, you will be liable, on conviction, to a fle of one thousand dollars and to imprisonment for six months. With regard to paragraph 3(c) of the Declaration, mere temporary absenc from the Colony (whether for business, holiday or any other rasmi during the three years preceding the date of this Application does no necessarily mean that you were not ordinarily resident in the Colony for those three years.
Note 5. Your attention is also drawn to the fact that, if you are registered, you will be registered in that part of the Provisional Register compiled for the electoral district in which you reside. This means that you will be entitled to vote only at the polling station designated for bil electoral district (as shown in the Provisional Register) unless the Revising Officer allows you to vole at another polling station. Should you wish to vote at another polling station and consider that you hav special grounds for so doing, you may so apply to the Registration Offer on the appropriate Form at any time after publication of thi notice of Provisional Register up to, and including, the
day 19 for if that day is a general holiday, then vp to, and including, the next day that is not a general holiday). Note 6. When you have completed and signed this Form, you may post it to
the Registration Officer. No stamp is required.
