do hereby apply to you for a licence, for the use and occupation as a slaughter- house of the premises hereinafter described; and I do hereby declare that to the best of my knowledge and belief the Schedule hereunto annexed contains a true statement of the several parti- culars therein set forth with respect to the said premises.
